 RMA Procedures

1. Please complete the RMA form.    (DOWNLOAD HERE)
   
2. FAX the completed RMA form with a copy of the original invoice to 2521 6118.
   
3. The RMA Dept. will FAX you an RMA number within 24 hours after receiving your fax or provide a reason for the RMA denial.
   
4. Please write the RMA Number on the outside of your shipping box.
